Allergy and Asthma Plans

Working with you, your child, the healthcare provider, and the school is the best way to keep your child healthy, in school, and ready to learn. If your child requires emergency medication for asthma and/or severe allergies during the school day, please complete one or both of the forms below and personally return the form and medication to the school.
